Statewide Program Expands to Help Minnesota Seniors

The Senior LinkAge Line®’s Return to Community initiative currently helps people move from a nursing facility back into a community setting.

Senior LinkAge Line® community living specialists work closely with the resident, their families and nursing facility staff to facilitate discharge and ensure supports are in place for the return home.

Return to Community will expand beyond nursing homes to help even more seniors in 2018, including those who:

• have discharged from a hospital who are within a set of socioeconomic criteria;

• have a doctor who ordered them to stay more than 90 days in a nursing home;

• are admitted to a nursing home on a respite stay;

• have decided not to move to an assisted living facility after discussing their options with the Senior LinkAge Line®, and/or

• have reached the end of Medicare-certified home care.

Before this life changing expansion begins, Return to Community staff will be holding a local community meeting on Monday, December 4 from 9-10:30 a.m. in the Merit Center, Room A, 1001 West Erie Road in Marshall. RSVP to Erica Schott at eschott@mnraaa.org.
