Prairie Home Hospice Receives Grant From Otto Bremer Trust

Prairie Home Hospice recently received a grant in the amount of $55,000 from The Otto Bremer Trust to support their nonprofit mission of serving the community through the delivery of quality hospice and home care services.
"We are so grateful for our partnership with The Otto Bremer Trust," said Pat Mellenthin, Prairie Home Hospice CEO.
"They understand the needs of our rural communities and the importance of the work we do in the community."
Prairie Home has been providing in-home hospice care in the community since 1984 and now provides residential hospice care at two locations, Lockwood Hospice House and McLaughlin Hospice House.
"We began providing home care services in 2014 because we saw a growing demand for in-home services and supports that allow people to remain living independently in their own home," said Mellenthin.
"With the acquisition of Prairie Home Fieldcrest in Cottonwood we now offer assisted living as another option for those who seek to balance independent living with a personalized plan of services. Our goal is to offer a variety of services and supports to meet a variety of needs and preferences."
Funding from The Otto Bremer Trust will be used in part to support our “No Place Like Home” program.
"We know that people want to remain living in their homes for as long as possible," said Tara Plante, Marketing & Development Director at Prairie Home Hospice. "This program offers the support they need to do that in a way they can feel confident about.”
"That may include providing assistance managing medications; monitoring blood pressure or blood sugar levels; light housekeeping; personal cares; help with coordinating health care appointments, etc. We’ll provide personalized services and supports to help support people in their own home, because we know – there’s no place like home."
